Application Checklist and Process

Process for Kindergarten – First Grade

  • Application: Submit a completed application for each child with a $250 non-refundable fee.  Once the application is received the Admissions Coordinator will schedule testing.  The testing assessment will take approximately 30 to 40 minutes.
  • Kindergarten through First Grade Teacher Recommendation Form: Submit to child’s current teacher and request they send or fax the completed form to Grace Christian Academy.
  • Kindergarten students that have attended preschool should submit a progress report.  First grade applicants should submit a Kindergarten report card.
  • Pastor/Youth Pastor Recommendation Form: Submit to the pastor of the church you currently attend.  Any minister on staff may complete the form and send or fax to Grace Christian Academy.
  • Student Health Form - complete and submit with application.
  • Family Enrollment Information - complete and submit with application.
  • Emergency Information - complete and submit with application.
  • Birth Certificate (Copy)
  • Immunization Record current (Copy)
  • A Family Interview will be scheduled when ALL of the above paperwork is turned in and testing results are available.  You will receive a decision letter two weeks after the interview.

Process for Grades 2-5

  • Report Cards (Copy)- students in grades 2-5 must submit a completed report card from the previous school year and a current report card.
  • Standardize Test Scores (Copy) - submit scores from the previous school year.
  • Second through First Grade Teacher Recommendation Form - submit to child’s current teacher and request he/she send or fax the completed form to Grace Christian Academy.
  • Pastor/Youth Pastor Recommendation Form - submit to the pastor of the church you currently attend.  Any minister on staff may complete the form and send or fax to Grace Christian Academy.
  • Student Health Form - complete and submit with application.
  • Family Enrollment Information - complete and submit with application.
  • Emergency Information - complete and submit with application.
  • Birth Certificate (Copy)

Process for Middle/High School

  • Application - submit completed application for each child with a $250 non-refundable fee.  Once application is received the Admissions Coordinator will schedule testing.  The testing assessment will take approximately 40 to 60 minutes.
  • An Applicant Questionnaire must be completed by the student and submitted with the application.
  • Report Cards (Copies)- submit completed report cards from two previous years of school and a current report card.
  • Standardize Test Scores (Copy) from the previous two years.
  • English Teacher Recommendation Form - submit to student’s current teacher and request he/she send or fax the completed form to Grace Christian Academy.
  • Math Teacher Recommendation Form - submit to student’s current teacher and request he/she send or fax the completed form to Grace Christian Academy.
  • Pastor/Youth Pastor Recommendation Form- submit to the pastor of the church you currently attend.  Any minister on staff may complete the form and send or fax to Grace Christian Academy.
  • Student Health Form - complete and submit with application.
  • Family Enrollment Information - complete and submit with application.

  • A Family Interview will be scheduled when ALL of the above paperwork is turned and the file has been review by the High School Administration.  The day of the interview the student will need to come prepared to write a short book report on a book they have recently read.  You will receive a decision letter two weeks following the interview.
